Review the Seven Dimensions of Wellness document, and follow the instructions below.

1. Read the descriptions of the seven dimensions of wellness, and then rate yourself in each category on a scale of 1 to

10 (a score of 1 = low, and a score of 10 = high).

2. Using the Goal Setting Template, fill in each column. Provide your personal rating (from above), and list at least one or

more SMART long-term wellness goals that you have for each of the seven dimensions of wellness. Long-term goals

should be specific, measurable, attainable, relevant, and include a timeline.

3. For each goal, list all of the barriers that prevent you from reaching your goals. A barrier can be an excuse that you

use, or it can be something that physically prevents you from reaching your goals. Be specific!
